and [Your Association Abbreviation] for the password. 

Ex: First Name: Thomas Last Name: Lynch 
Username would be: 'ThomasLynch' (capital T and L)

Also, the initial password sent to you should be all lowercase. 

You will be prompted to change your password after you login.
